How Scheduling Works
Step 1: Request Your Appointment
Select your preferred appointment date and time online. A valid credit card is required to request an appointment but will not be charged immediately.
Step 2: Complete Your Initial Forms
You’ll receive a secure link to complete your required initial paperwork, including:

✓ Client Intake Form
✓ Credit Card Authorization
These forms must be completed within 24 hours of requesting your appointment. If the required paperwork is not completed within 24 hours, your requested appointment date and time will be released and become available for other clients.
Step 3: Appointment Confirmation
Once your required forms have been received, your appointment will be confirmed. At that time, your payment will be securely processed using the card you provided.
*Please Note: Appointment requests are not considered confirmed until the required initial paperwork has been completed and payment has been successfully processed.
Step 4: Complete Your Intake Packet
After your appointment is confirmed, you’ll receive access to your remaining intake documents. Please complete these forms at least 24 hours before your scheduled appointment.
Step 5: Attend Your Evaluation
At your scheduled appointment time, you’ll meet with your evaluator through our secure telehealth platform. Most evaluations take approximately 60 minutes.
